One of the most popular cosmetic surgeries, in the western world, is augmentation mammoplasty (breast enlargement).

Throughout the world, there is a growing demand for this surgery, and it runs a close second to liposuction in frequency, worldwide. It is also one of the many satisfying cosmetic procedures, in terms of patient satisfaction, within Dr. Barsky's vast practice.

Every woman wants to have a well-proportioned, pleasing silhouette. Classic beauties of the world have had firm, lifted breasts with balance and symmetry, to the waist and hips. There is no question that aesthetic surgery of the breasts, can result in a more pleasing appearance, and a rebirth of self-confidence.

Part of lives challenge is to feel a rich sense of acceptance. Breast augmentation surgery
alone will not accomplish this, but it does contribute to the overall satisfaction of feeling more feminine and lovely, which is why so many women seek the procedure in today's society.

Two groups of women generally seek breast augmentation. The first is made up of women in their late teens or early twenties, who have never enjoyed well-proportioned bosom. What Breast Surgery Accomplishes

Breast implants will make smaller breasts larger and fuller. At the time of examination, you will be asked what it is you wish to achieve with breast enlargement, in terms of size and shape. Dr. Barsky will have some questions for you, and point out possibilities and any limitations. Listen carefully. Make mental and written notes of the discussion. Dr. Barsky will indicate where the incisions will be placed, what type of implant will be used, and he will show you samples of implants. At the examination time, Dr. Barsky will explain the best approach to augmentation, customized to your special needs, desires, body shape, and structure.

Patient Choice of Implant

Allow Dr. Barsky to indicate what he thinks is best for you. Occasionally after surgery, patients wish that they had selected a larger or smaller implant. There are some who insist on smaller implants, which will not interfere with their active lifestyle. The jogger or athlete may want her breasts smaller for sports reasons, yet would like to fill out an evening dress or swimsuit. The choice may be difficult.

The common question, regarding size of implants, is, what size will look best for your body type? Ask yourself what size you would like to be. Do you want to fill a "B", "C", or "D" cup? In part, the answer to the question depends on how much breast tissue you already have. This may even be different for each breast. It is very common for a woman to have different sized breasts. Dr. Barsky has been known to re-contour many women's breasts, so that both sides become beautifully identified as a well matched pair.

Occasionally, a patient will want to choose implants that are too large for her body. In this case, they may be so large that they overstretch the skin. The unfortunate result of overstretching might be unattractive stretch marks. This is why it is important to listen to the advice of Dr. Barsky. Also, the smaller the implant, the less problem you may have with formation of capsule contractures (unusual firmness). Also, the possibility of distortion is less, with the same amount of capsule formation, when the implants are smaller.

On the other hand, you want the implant large enough to achieve full breasts. This is why Dr. Barsky's suggestions are of the utmost importance, in determining the proper size of the implants.

Be open with Dr. Barsky. It is important that you communicate your desires as well as your fears. Feel free to ask any questions you may have. Listen carefully to the answers and verify that you understand, and that the comments make sense to you. You should leave with all questions answered, and a clear idea of what to expect.

Again, in cases of unequal breast size, Dr. Barsky can minimize this flaw. A correction of asymmetry in volume, combined at times with a change in position of the inframammary fold (the underside of the breast on the smaller side), can give a pleasing balance to the breasts. Other asymmetries, such as inverted nipple, large areola, or ptosis (drooping or sagging), require additional surgery for correction, in addition to the breast augmentation discussed above.

Breast Implants

There are several implant manufacturers currently available. They offer the safest, most natural-appearing implants. Some implants are fulled with a saline solution, a gel, or a combination . In 1963, the silicone gel implant was invented. The bag consisted of a teardrop-shaped silicone membrane, filled with a firm gel. As early as the 1960's, it was discovered that silicone liquid injections, for the breast augmentation, led to unacceptable results, fraught with complications.

Saline-filled implants were introduced in the 1970's. Saline and gel implants are manufactured with both a smooth and a textured surface. Dr. Barsky will, of course, advise you as to which is right for you. In addition, there are two possible locations for implants in the breast; beneath the pectoral muscle, or between the muscle and the breast tissue. This issue is resolved based upon the size of the original breasts presented.

Today the technology of implants has greatly improved. Saline filled implants are available for initial augmentation. Improved, natural feeling silicone gel implants, are available for reconstruction and repeat augmentation. Extensive research on silicone gel implants have found no harmful effects from their use. However, Dr. Barsky - the ultimate perfectionist - prefers saline fulled implants for his patients.

Dr. Barsky is not a gambler, especially when it comes to the health and well being of his patients.

Sites of Incision

As a rule, Dr. Barsky selects the site of incision. There are at least three entry areas to the breast, in order to insert the implant. Dr. Barsky will discuss the site of implant entry, and may even offer a choice, but it is wisest to follow his recommendation for best results. As with incision location, placement is a technical question best decided by Dr. Barsky.

Transumbilical Breast Augmentation

Transumbilical breast augmentation is performed on an outpatient basis, utilizing general IV sedation. Dr. Barsky's technique for performing breast augmentation, most often, is through an incision in the navel. Utilizing this technique, no incisions are made on the breast, or in the breast tissue.!!

Dr. Barsky makes a small incision in your belly button. Through this incision, Dr. Barsky passes a slender instrument under the skin, to a location behind the breast tissue. Next, a tissue expander is inserted, and placed under the breast. With this expander, a space is developed to create a "pocket" for the implant size that you've requested. The position and size of this pocket is verified. Then, an empty saline implant is placed in the pocket, and fulled with sterile saline. Dr. Barsky can adjust the amount of saline in injected into your implant, based on the size you have selected. This helps to correct any asymmetry of your breasts!! The incision in your navel is then closed with a few absorbable sutures!! Bingo - you're voluptuous!! Zap - you're beautiful!!

What Are the Advantages of Transumbilical Implants, As Performed by Dr. Barsky?

What is Recovery From Transumbilical Breast Surgery Like?

Rest is recommended for the first two days following surgery. A post-surgical sports bra is worn for several weeks. Mild discomfort associated with breast surgery can be controlled with oral medication, prescribed by Dr. Barsky. Antibiotics are also prescribed. Many women experience mild swelling, bruising, and tenderness. These symptoms do subside in a short time. Most of the time, sutures in the navel are dissolvable, and require no removal. Dr. Barsky will advise you on a proper schedule for returning to normal activities. Most patients are back to work within a few days!! Regular physical activity may be resumed within 2 to3 weeks, working your way up gradually - let your body dictate your ascent into physical exercise.

Hematoma

Hematoma is a pooling of blood and fluid, and occurs mire often in patients who have taken blood thinners, like aspirin, prior to surgery. In addition, hematoma may occur in patients with elevated blood pressure. Other causes are coagulopathies and excessive heat, or exertion, after surgery.

Furthermore, hematomas may be the result of a blood clot around the implant, due to a broken blood vessel. Should this complication arise, it occurs within the first week or so following surgery and most often immediately after surgery. Be assured that this is not life threatening, either as a blood clot, or from blood loss. It is what is termed a "closed space" hematoma.

In the rare incidence that a hematoma develops, the patient will notice swelling of one of the breasts, accompanied by tenderness, or perhaps firmness and bruising, or discoloration. Small clots can result without any of these usual signs. The rare patient who may need surgical evacuation of a hematoma will usually be uncomfortable, with a hard, swollen, breast on one side. Any patient with these signs needs to see the doctor immediately. This condition can be cleared up with proper medical care.

Noticeable Scars

Most incidences of scar revision include those patients with almost invisible scars, but who feel they are noticeable. Whether or not they are noticeable is a matter of opinion. To most, the beneficial results from breast augmentation far outweigh any minute scar, if any.

Asymmetry

All breasts are to some extent different. Nature made no two snowflakes, grains of sand, or anything else a human can see, exactly alike. Whether or not there is significant asymmetry, again, is a matter of opinion.

Many asymmetries exist prior to surgery. Dr. Barsky will correct what is visible, and make the two breasts as symmetrical as possible during surgery. Dr. Barsky may also be able to improve abnormal shaping of the chest with implants, as he has proven on
numerous occasions!!!
